We are honored to accept the 2013 Expanding the Vision Grant through the Meals On Wheels Association of America and the Walmart Foundation. We will receive $94,439 toward reducing re-hospitalizations and improving care of recently discharged patients by providing meals within Tarrant County.
The target audience is recently discharged (within two weeks) hospital patients or emergency room patients age 60 or older residing in Tarrant County. The average hospitalization cost within Tarrant County is $25,000. If we meet the goal of reducing hospitalization of 60 clients within the service period, it could potentially save $1.5 million, which is a return on investment of 1,400%.
